OCR Prompt Issue: Unexpected Underscores, Hyphens, or Missing Spaces in Output

I set the prompt as:
qs = 'OCR: Please read the text in the image and insert appropriate spaces between words.'
However, the result often replaces spaces with underscores or hyphens, or sometimes doesn’t insert any spaces at all.
Is this considered a hallucination and is it a common behavior?
If anyone has any suggestions on how to fix or improve this, I would really appreciate it.
